Checklist

Make sure that you are ready to begin classes. The following checklist indicates what you need go do to be prepared.

Submit Health Forms

Send your completed “Report of Medical History” form to Student and Employee Health, 29 South Paca Street, Baltimore, MD 21201. The Web site is www.umaryland.edu/health. If you are covered by a health insurance policy, you must complete a health insurance waiver, found at the above web page.

Submit Copy of CPR

Submit a copy of your CPR card Professional Rescuer (without automatic defibrillator) or Professional Healthcare Provider-level CPR certification. Certification should be taken no more than 3 months before the first day of class. (Certification must be current throughout the program.)

Obtain a Parking Permit

The student parking lot is located at Lexington Garage, 680 W. Lexington Street. Parking permits are processed through the Web site at www.parking.umaryland.edu

Obtain your OneCard (Student ID)

Obtain a signed Photo Identification Authorization Form from the Office of Registration Services, in room 108. Take the signed form and one additional piece of photo identification – such as a driver’s license – to the Photo Identification Office of the Pine Street Police Station, Pine Street Police Station, 214 N. Pine Street, 1st Floor. Hours of Operation: Monday, 10:00 a.m.- 2:00 p. m., Wednesday, 12:00 noon - 4:00 p.m., Friday, 10:00 .m. - 2:00 p.m. The Web site is www.umb1one.umaryland.edu/.

Set Up an E-mail Account

If you wish to access the University's main server and the Library's databases from your home computer, go to www.umaryland.edu/helpdesk/apps. Applications must be printed out and signed. They can either be faxed or mailed to: Health Sciences and Human Services Library, ASC Help Desk, Room LL06, 601 West Lombard Street, Baltimore, MD 21201- 1512, or Faxed to 410-706-4191.
